This questionnaire will help me start getting to know you and your business better so I can provide the support you need to achieve your goals.
Please take your time answering each question thoughtfully but also, try not to overthink it. Feel free to share as much or as little as you feel comfortable.
There is no rush on this, but please complete it before our onboarding call.

1. Have you ever worked with a coach, mentor, or counselor before? If so, what was your experience like?

2. What other personal or professional development work have you done (books, programs, courses, etc.) and what has been the most helpful for you?

3. If someone gave you 10 million US dollars (enough to live off the interest), how would you spend it? How would you spend your days if you no longer needed to work to earn an income?

13. Your mentorship program includes Voxer support, which you're welcome to use as much or little as you like. If I don't hear from you in between our calls, what kind of support would you like?
